Pre-Qualifications
Minimum Age	At least 21 years old
Minimum Experience (tractor Trailer/OTR)	At least 12 months verifiable Tractor Trailer experience in the past 3 years  Trainers must have 12 months verifiable Tractor Trailer experience in the past 3 years  *All Tractor Trailer experience is counted exceptions – Logging, Oilfield, Farms  * Training experience will count towards experience, but not the TDS portion  * MUST have a valid CDL in the state of residence before applying! * TEAMS: Drivers MUST live in the same state!
Moving Violations (max in 3 year period)	No more than three (3) moving violations in the past three (3) years No more than two (2) in any 12-month period No more than one (1) in the most recent 12-months No DWI/DUI Convictions in the past (3) years No truck abandonments at unauthorized location in the past three (3) years * All pending violations must be closed before a driver can drive for DBS
Moving Violations – NONE allowed in the past 3 years (Listed on the Driver’s MVR)	–Under the influence of drugs or alcohol including refusal to test –Leaving the scene of an accident –Causing a fatality through negligent operation of a CMV –Driving while CDL is suspended, revoked or cancelled –Using a vehicle to commit a felony –Railroad-Highway Grade crossing violations –Violating out of service orders –improper or erratic lane change
Moving Violations – 1 allowed in a personal vehicle the past 3 years – None in a (Listed on the Driver’s MVR)	–Speeding 15+ –Reckless driving –Following too close –Driving a CMV without obtaining a CDL –Driving a CMV without a CDL in possession –Driving without proper class or endorsements for cargo being hauled –Texting and/or handheld mobile violation
Serious Traffic Violations – None allowed in the last 3 years (Listed on the MVR)	-Leaving the scene of an accident -Causing a fatality thru negligent operations of a CMV -Driving while CDL suspended/revoked/canceled -Using vehicle to commit a felony -Railroad-highway Grade crossing violations -Violating out of service orders
Policy on License suspensions	Will review (DQ for driving on a suspended CDL)
DOT Recordable Accidents	-No more than 2 preventables in the past 3 years -No preventables with a serious traffic violation (listed above) -No DOT reportable preventable rear ends -No Rear ends resulting in a serious traffic violation (listed above) -No Jackknifes -No U Turns (resulting in an accident) -Rollovers MUST be outside of 3 years
Collisions	No Major preventable collisions resulting in any one of the below in the past three (3) years: -Any preventable collision with a serious traffic violation -DOT reportable rear-end or rear-ends resulting in a serious traffic violation -Rollovers/Overturns -Jackknife -U-Turns No more than two (2) preventable collisions in the past three (3) years
PSP	-No more than 90 points in Vehicle Maintenance – CSMS -No more than 90 points in Hours of Service – CSMS -No more than 100 total points – CSMS * Cannot have any violations pertaining to drugs and/or alcohol in the past 3 years
Incidents (max in 3 year period)	Will be considered for the overall review
Max. Number of Jobs- in the last 3 years	-No more than 3 jobs in the past year -No more than six (6) driving jobs in the past three (3) years (including orientation only) -No terminations from most recent previous employer for cause
Policy Against Unemployment- even if it is accounted for	If unemployment is listed, and is over 30 days, details for what the driver was doing is required
Terminated Applicants	Cannot be termed for cause (something negative) from most previous driving job
Felony Convictions – How old must they be? Based off the conviction date? What are the “silver bullets”?	-None in the past 7 years for sex, drugs, theft/fraud or violence  -Never if-active on the sex offender registry, human trafficking or acts of terrorism convictions -No registered sex offenders  * All other charges will be reviewed * Go by conviction date * Drivers on probation/parole will be reviewed and will be required to submit documentation if approved
Misdemeanors- How old must they be? Automatic DQ’s?	-None in the past 3 years for sex, drug, theft/fraud or violence. -No registered sex offenders  * All other charges will be reviewed * Go by conviction date * Drivers on probation/parole will be reviewed and will be required to submit documentation if approved
DUI/DWI- What is the max number and what time frames do they need to fall outside of?	Must be outside of 10 years (Conviction Date)
Is Haz-Mat Required? Is there a grace period for driver to obtain it?	HazMat not required
Reimbursed for Haz-Mat?	NO
Haz-Mat Pay Info	$0.03 cpm more on all HazMat loads
Other Endorsements Required	Some Dedicated runs require endorsements
Long Form Physical Required Up Front?	Yes If the driver doesn’t have the long form and medical cert, and at least 6 months on their card, they will have to complete a physical in class.
DOT Physical Requirements	Existing physicals must have at least six (6) months left on expiration. Driver must provide a long form and medical card upon arrival to orientation. MVRs ran pre-hire must be checked for valid medical certification. Any MVR with a medical certification less than 30 days of expiration must be renewed prior to the driver being brought in for orientation. Physicals resulting in a less than 12-month card for Obstructive Sleep Apnea and/or sleep study requirements may be denied.
Drug Screening	Urine
Other Automatic DQ’s.	-No truck abandonment at an unauthorized location in the last 3 years -Have no record of positive drug and/or alcohol test with previous employer in the past ten (10) years -Have no record of adulterated, substituted or refused drug and/or alcohol test with previous employer in the past ten (10) years
PSP	-Will be scored by USAT -Cannot have any violations pertaining to drugs and/or alcohol in the past 3 years -PSP violations will not count toward citation totals, unless the PSP violation is listed on the MVR

Orientation will be held in Atlanta, GA; Dayton, OH; Memphis, TN or Dallas, TX beginning on Monday if the driver needs a physical and Tuesday if they do not.
Travel, lodging and meals will be provided, details discussed during the application process.
